SUBJECT: Letter of Reprimand
1. You were recently counseled verbally regarding your wish of ray of light control stemming from you leaving your headset unattended on a launch ready aircraft and and then failing to nonify your supervision. Since then you again failed to maintain positive control of your headset and instead of stateing person you resorted to signing out headsets while fully aw ar that they were missing. This is the uphold time in a week that you have failed to not only maintain control of your equipment but your failure to notify anyone cannot be tolerated. You will therefore surrender your headset to CTK and no perennial be issued an some other pair until you can show that you be responsible.
2. You are hereby reprimanded; your failure to adhere to proper tool control cannot be tolerated. Your family, your team, this unit, and indeed the entire Air Force, cannot figure out when members cannot maintain acceptable tool control. A repetition of this unacceptable behavior will result in stronger action against you.
